A standard inquiry form comes with every site. You can add your own questions. Professional sites can have multiple forms (polls, questionnaires, surveys...).
The Web Deacon™ provides a standard inquiry form with each site. In its native form it just asks for contact data and how the organization might be of service. You can also replace the default question with questions of your own.

All sites also come with an optional guest registry. And of course the editing forms are part of the system for all users.

Premium (upgraded) sites can have multiple forms. Use your additional forms for polls, surveys, questionnaires... The built-in forms editing tool provides all standard data entry modes (text, radio buttons, popup lists...).

Premium sites also are eligible to use the Roster and Directory systems, which have their own data entry forms and forms visitors to the site may use to send messages to persons or other listed entities. This prevents spam, as the related email addresses are not published.

If necessary we can customize a database to store your results. This is, of course, subject to a design fee and monthly storage and transmission fees, depending on the volume of usage.
